The Guest Editor introduces this Theme Issue of the Schizophrenia Bulletinwith an overview of the kinds of subject matters appropriate for a comprehensive understanding of thevital statistics and demography relating to schizophrenia and schizophrenic patients—births,deaths, marriages, divorces, fertility, age, sex, race, occupation, place of residence, season of birth,class mobility, homelessness, criminality, industrialization, and suicide. Major sources of thestatistics relating to schizophrenia are listed. He calls for a national case register and the use of aunique identifier with linkage across data bases in various branches of government so as to inform adescriptive as well as an experimental (hypothesis-testing) epidemiology.

The hypothesis ispresented that the etiology of schizophrenia is neurodevelopmental: schizophrenia is a disorderoccurring in extremely late maturers, whereas manic-depressive psychosis affects earlymaturers. This hypothesis is related to recent neurobiological findings and also to thefollowing epidemiological and demographic topics covered by the author in her review of socialclass, marriage, and fertility in schizophrenia: Kretschmer’s observations of bodytype differences between patients with schizophrenia and manic-depressive psychosis; trends inthe incidence of schizophrenia and manic-depressive psychosis in industrialized versusdeveloping economies; changing epidemiology of the subtypes of schizophrenia and ofmanic-depressive psychosis; sex differences in manic-depressive psychosis and schizophrenia;fertility and childlessness in schizophrenia; selection for marriage in schizophrenia; marriagepatterns, inbreeding, and schizophrenia; social class, social mobility, and occupation inschizophrenia; social mobility and social selection; excess of schizophrenia in the loweststrata of society; social class, course, and outcome; and social stress and schizophrenia.
