Ophthalmoplegic Migraine or Tolosa‐Hunt Syndrome? Letters to the Editor Computed Tomography in the Routine Evaluation of Headache Mauro Silvestrini, M.D., Giorgio Bernardi, M.D. Clinic of Neurology, "Tor Vergata" University of Rome IRCCS, "S. Lucia", Rome, Italy The value of computed tomography (CT) in the evaluation of patients with headache was discussed by Mitchell et al. in the February 1993 issue of Headache. Based on their results and on data of previous studies, they concluded that in headache patien ts with typical clinical symptoms and with normal physical and neurological examination, routine brain CT has a very low yield for discovering significant intracranial disease. Hence, based solely on cost-effectiveness of CT there should be a dramati c reduction of examination by CT in headache patients. However, as stated in the Editorial by Campbell in the same issue of Headache, the strict adherence to a purely statistical point of view has the potential hazard of overlooking several importan t aspects of clinical practice. In other words, the a priori neglect of the importance of neuro-imaging in headache patients could hinder the diagnosis of cerebral lesions that have not yet caused focal signs.
