Pathology of Tumours
mode of origin of carcinonla in Lutaneous and visceral sites has been the subject of reappraisal in recent years.9 It has been shown that a considerable proportion of cancers in the skin,"? 2 5 stomach,5 l ~ n g , ~ ~cer-7 -l vix,2 and bladder14 are accompanied by, and indeed may arise froin, multiple primary independent loci. I n the process of expansion, many small neighboring tumors may coalesce, resulting in varying histological patlerns in what appears to be a single neop1asm.O Grossly imperceptible independent areas of both preinvasivc and early invasive carcinoma are often encountered at varying distances from presenting major neoplasms.l,4 I n many smlace tumors, plaques o preinvasive carcinoma have f been noted in contiguitv with the margins of the primary lesions and have been observed to extend peripherally lor considerable distances. Some have considered this to represent superficial spreading carcinoma.10,l2 0thers, however, have felt that lesions of this nature have pre-cxisted the manifest cancer and indecd represent a source for it5 initiation. Similar considerations appear applicable to the growth and development of carcinoma of the breast.13 Several investigators, notably Cheatle and Cutler,3,4 have demonstrated several instanres of breast cancer with multicentric character.
